We’re looking for Medical Assistants and LPNs to join our Adult Day Health Care & Home Health Care (PACE®) team in Tacoma, Washington. If you have 1 year of acute care nursing, case management or patient care coordination or experience working with frail, geriatric or disabled adults, and are looking to join a tight-knit and collaborative team with a mission-driven organization, we want to hear from you! The Program of All-Inclusive Care for the Elderly (PACE®) is a comprehensive, fully integrated, Provider 1 based health plan for the frailest and costliest members of our society – those who require a nursing home level of care. The PACE philosophy is centered on the belief that it is better for frail individuals and their families to be served in the community whenever possible. PACE programs are focused on delivering collaborative approaches to care and services that allow participants to live at home safely.
